A heavy lower face is often the first sign of structural change.

One of the most common concerns we treat at Eleventh Heaven™ is the loss of definition along the mandibular border—commonly known as “jowling.”

Many patients believe the only solution is to add volume to the jawline to hide the sagging. However, simply adding volume to a heavy lower face can often make it look wider, rather than youthful.

We do not simply camouflage the jowl; we aim to treat the cause of the descent. By combining deep structural support with skin tightening and muscle-lifting techniques, we restore the clean, crisp silhouette of the jawline without the “heavy” look.

The Anatomy of Sagging - Why Jowls Form

The “slide” of the lower face is caused by changes in three distinct layers. We must address all three for a natural result.

1. The Pre-Jowl Sulcus (Bone Loss)

The jawbone naturally shrinks with age, specifically in the area between the chin and the jowl. This creates a “dent” (the pre-jowl sulcus) that makes the jowl fat look prominent by comparison.

2. Skin Laxity (The Envelope)

As collagen degrades, the skin loses its ability to “snap back” against the bone. It begins to drape loosely, pooling at the lowest point of the jaw due to gravity.

3. Muscular Pull

The large muscle of the neck (Platysma) pulls the lower face downward. Over time, this constant downward traction blunts the sharp angle of the jawline, merging the face into the neck.

The Eleventh Heaven Protocol

We rarely use a single modality to treat jowls. We use a “Sandwich Technique” to address bone, muscle, and skin.

1. Structural Volumisation (Deep Support)

We place a structural volumising treatment deep on the bone in the “Pre-Jowl Sulcus.” By treating the hollow in front of the jowl and strengthening the jaw angle behind it, we straighten the jawline visually without making the face look wider.

2. Thread Lifting Procedures

For patients with true tissue descent, volume alone is not the answer. Dr Ricky may recommend a Thread Lift procedure to physically reposition the sagging fat pads back to their original position, aiming to reduce the weight of the jowl.

3. Energy-Based Tightening

To “shrink wrap” the skin against the new contour, we use Ultherapy PRIME or broadband light energy to stimulate new collagen production, firming the tissue envelope over time.

4. The “Nefertiti” Muscle Release

We use precise treatments along the jawline to lessen the downward pull of the neck muscles. This releases the tension, allowing the facial muscles to lift the jawline back up naturally.

Will jawline treatment make my face look wide?

This is a common fear. If volume is placed incorrectly (on the side of the face), yes. However, we place product on the topand edge of the bone to create definition, not width. Our goal is a contoured look, not a heavy one.

Can you dissolve the jowl fat?

We focus on lifting and camouflaging. If you have significant sub-mental fat (double chin), we may recommend fat-reducing therapies or refer you for surgical options if that is the superior medical pathway.

What is the difference between Jowl treatment and Chin Volumisation?

They work together. The chin is the “anchor” of the lower face. Without a strong chin projection, the jawline has no ending point. We often treat the chin first to provide the tension needed to smooth the jowl area.
